Community Health Program 

This program is facilitated by the Community Health Representative (CHR)

The CHR in conjunction with our heath staff provides health promotion, health maintenance, injury and illness prevention within the community to all ages and stages. The program and supports are offered through community engagement, awareness and education, either through group or individual sessions.
